Glossary Primary Care Services                   

Term

Meaning

Condition Management Indicators

•        Measures, called indicators are agreed as part of the GP contract              negotiations every year. These indicators have points attached                  that are given to GP practices based on how they are doing                        against these measures.  As an example –
•        management of some of the most common chronic conditions,               for example asthma and diabetes
•        management of major public health concerns, for example                       smoking and obesity
•        providing preventative services such as screening or blood                         pressure checks

CQC

CQC is the independent regulator of health and adult social care in England.

They make sure health and social care services provide people with safe, effective, compassionate, high-quality care and they encourage care services to improve.

CQC Inspection

CQC conduct announced and unannounced inspections across all areas of the NHS and Social Care.  These results are used to ensure improvement and allow users to make sure that care givers are trained and adequately resourced.

Effective Management of Long Term Conditions

Where different ranges of measures are used to record individual indicators these are standardised to enable us to view the overall performance across the Management of Long Term Conditions as a whole or as a selected range.

Full Time Equivalent

The calculation of full-time equivalent (FTE) is an employee’s scheduled hours divided by the employer’s hours for a full-time workweek. When an employer has a 40-hour workweek, employees who are scheduled to work 40 hours per week are 1.0 FTEs. Employees scheduled to work 20 hours per week are 0.5 FTEs.

This is done to standardise reporting.

GP Practice

General practitioners (GPs) treat all common medical conditions and refer patients to hospitals and other medical services for urgent and specialist treatment.  The Practice is the business which the GP is employed by.

 

ICS

Integrated care systems (ICSs) are partnerships that bring together NHS organisations, local authorities and others to take collective responsibility for planning services, improving health and reducing inequalities across geographical areas.  England is split into 42 ICS’s.

In this instance these are the Long Term Conditions for example Diabetes, Heart Failure, Mental Health etc.

IPSOS MORI

IPSOS are a Market Research Company who are responsible for the  latest GP Patient Survey (GPPS) . This report details the findings of patients’ experience of their local GP practice and other local NHS services.

IPSOS MORI Patient Survey

The GP Patient Survey (GPPS) reports details the findings of patients’ experience of their local GP practice and other local NHS services.

The survey asks about patients’ local GP services (including use of online services, awareness and satisfaction with opening times and preferred GPs), experience of making an appointment, the quality of care at their last appointment, overall experience of their GP practice, experience when their GP practice is closed and NHS dentistry as well as current health circumstances.

Local Authority

A local authority is an organisation that is officially responsible for all the public services and facilities in a particular area.

Long Terms Conditions

 

                                       

A long-term condition is an illness that cannot be cured. It can usually be controlled with medicines or other treatments. Examples of long-term conditions include diabetes, arthritis, high blood pressure, epilepsy, asthma and some mental health conditions.

LSOA

LSOAs (Lower-layer Super Output Areas) are small areas designed to be of a similar population size, with an average of approximately 1,500 residents or 650 households. There are 32,844 Lower-layer Super Output Areas (LSOAs) in England.

National Average

The average result when combining all geographic areas in England.

Nurse

General practice nurses work in GP surgeries as part of the primary healthcare team.

Pareto

This is a chart type that are often used to identify areas to focus on first in process improvement. Pareto charts show the ordered frequency counts of values for the different levels of a categorical or nominal variable.

PCN

Primary care networks  – PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care .

Prevalence

Prevalence is the proportion of a population who have a specific characteristic in a given time period.

Primary Care

Primary care services provide the first point of contact in the healthcare system, acting as the ‘front door’ of the NHS. Primary care includes general practice, community pharmacy, dental, and optometry (eye health) services.

Single Indicator

Measures, called indicators are agreed as part of the GP contract negotiations every year. These indicators have points attached that are given to GP practices based on how they are doing against these measures.

These Indicators are themed in line with common Long Term Conditions but then broken down into relevant tasks related to that condition,  For example –

A register of patients with asthma, excluding patients with asthma who have been prescribed no asthma-related drugs in the preceding 12 months

The percentage of patients aged 8 or over with asthma  on the register, with measures of variability or reversibility recorded between 3 months before or any time after diagnosis

The percentage of patients with asthma, on the register, who have had an asthma review in the preceding 12 months that includes an assessment of asthma control using the 3 RCP questions.

Trend

Trend charts are used to show trends in data over time.
